How to Make an Appointment
If you would like an appointment or a second opinion from our Neurosciences team, ask your child’s primary care provider for a referral.
To help us prepare for your first appointment and coordinate your child’s care, ask your doctor to fax us your child’s clinic notes with a New Appointment Request Form.
In addition to Seattle Children’s seven Washington locations, our neurologists see patients at community clinics in Missoula, Montana, and in Bethel, Juneau, Ketchikan and Sitka, Alaska. Call 844-935-3467 (toll-free) for more information.
