Seattle Children’s Hospital welcomes international patients. To provide the best care for your child, we will carefully review your child’s medical records to choose the best treatment and care.

Please complete this form and submit it with the requested medical records (translated into English) and supporting documents listed below.

Once we receive the intake form and the child’s medical records we will review the request and determine next steps.
