If you are a patient or a family member of a patient and no longer wish to receive fundraising materials, please fill out the information below. All required information is needed to successfully opt out of receiving fundraising materials. We will not use the information submitted for any other purposes.

Completing this form opts the patient or family member out of receiving fundraising materials only. You may be contacted by Seattle Children’s for other purposes.

You will in no way impact your care or that of a family member by opting out of receiving fundraising materials from Seattle Children’s. You will still be contacted in the same way by your medical care team regarding any medical information.
