How to Make an Appointment at the Heart Center

To make an appointment at any of the Heart Center’s locations in Washington or Alaska, you can call directly or get a referral from your child's primary care doctor. We encourage you to work with your primary care provider or family physician when coming to Seattle Children’s specialists.

How to Refer a Patient to the Heart Center

If you are a provider, fax a New Appointment Request Form (PDF) (DOC) to 206-985-3121 or 866-985-3121 (toll-free). Read more about referring to the Heart Center.
