Health Information Management and Medical Records Requests

Health Information Management maintains all of our patients’ medical records. Contact us when you want to request copies of your or your child’s health information.

How to Request Medical Records

At this time, our office does not allow in-person deliveries. Please follow the instructions below to submit your request.

  1. Patients or parents/legal guardians fill out forms.
  2. Send the form to Seattle Children’s Health Information Management:

Your records will be mailed within 15 business days after Health Information Management receives the form.

You have the right to cancel your Authorization to Release Patient Health Information. To do so, complete the Revocation of Authorization Form (PDF) and send it to Seattle Children’s Health Information Management.
